Enrollment Intake, & Candidacy 


R-5 High School provides an alternative educational choice setting open to all students. However, as a school of choice, space is limited and all prospective students must apply for entry. As an alternative school, R-5 offers a narrower range of elective choices than found in a traditional high school. The application and enrollment process is designed to ensure a match between the school's program and course offerings and the student's needs.

For the 2019-2020 school year this packet needs to be filled out and brought into R5 High School by  August 6th.  Any applications taken after the August 6th date, the student will be considered for next Intake date September 18th. Starting July 23, 2019 parents need to fill out an online registration form here on ParentVUE. If you do not have a ParentVue, you will need to create one in order to register. Parents who have one can login and fill out the registrations. Once completed you MUST call the R5 High School Registrar Melissa Smith at 970-254-6880 to let her know you completed your registration and what school it was automatically sent to. If you have a printer, please print these forms, fill them out and bring them to the front office. If you do not have a printer you can come to the school and fill the forms out. This does NOT include registration forms, these must be done online. If your child is 18 or older, and they live at home you must still complete the online registration for them.

Students who are 18 or older and live on their own must come into the school to fill out ALL registration forms. 


Once the completed application is on file and it is determined that the student meets the R-5 criteria, then a R-5 secretary will schedule an interview with our counselor. During the interview, we will assess the student's educational needs and goals. The student will have the opportunity to explain why they wish to attend R-5 High School, their goals and aspirations, educational experiences, and academic history. R-5 offers multiple intake dates throughout the year (Aug 14, Sept 17, Oct 1, Oct 16, Jan 8, and March 12), and the student will be invited to begin the first day of the next available intake day.


Upon entering R-5, students must have a signed training agreement for a job or volunteer position, be enrolled in Career Center or Western Colorado Community College, or be enrolled in a 7th and 8th hour vocational intervention at R-5. Students are in candidacy during their first ten days at R-5. During Candidacy, the student must demonstrate commitment to his/her education at R-5 by physically attending all classes a minimum of 90%, actively engaging in each class, demonstrating readiness for R-5 and participating in vocational training. More than one absence in this ten day period will bring the student below 90% attendance and may result in not meeting candidacy. Failure to meet candidacy results in the student losing his/her status as an R-5 student. Students who do not meet the candidacy requirements may be invited to reenter on candidacy at the next intake day and/or may always reapply at a future date. Upon successful completion of Candidacy, the student is an official member of the R-5 student body!
